Skilled Nursing Services

AHCP provides skilled nursing care services through registered nurses and licensed practical nurses. Registered nurses provide evaluation and treatment of acute and chronic medical conditions.

Skilled Services include:

  • Client Assessment
  • Wound Care/Dressing Changes
  • Foley Catheter Insertion and Care
  • Injections
  • Patient Education
  • Pain Management
  • Clinical Case Management
  • Post Surgery Care

Physical Therapy

AHCP’s licensed physical therapists provide evaluation and treatment of clients experiencing acute onsets or flare-ups of various neurological, bone, or muscular disorders. Based on a complete evaluation, our physical therapists develop treatment plans in conjunction with the client’s physician and use a variety of treatment techniques.

Physical Therapy Techniques include:

  • Exercise programs for strength, endurance, coordination, and balance
  • Walking and transfer training
  • Instruction in body mechanics and safety
  • Wellness education
  • Patient Education
  • Pain Management
  • Clinical Case Management
  • Post Surgery Care

When appropriate, our physical therapists provide family or caregiver training to reinforce and enhance the benefits of treatment.


Occupational Therapy

AHCP’s licensed occupational therapists provide activities and instruction designed to increase safety and independence in daily activities, as well as functional use of the client’s arms and legs.

Treatment may include:

  • Evaluation of and fitting for adaptive equipment
  • Instruction in energy conservation, work simplification, and joint protection
  • Perceptual motor training
  • Intervention for psychiatric disorders
  • Adaptive training for successful completion of daily living activities
  • Focus on developing each client’s functional skills, tailored to meet the specific needs of each disability

Speech Therapy

AHCP’s licensed speech pathologists are an integral part of the rehabilitation team, with expertise in the management of communication and swallowing disorders. Treatment focuses on increasing each client’s communication skills and the oral-motor skills needed for safe swallowing.

Treatment is designed to improve any or all of the following:

  • Speech characteristics
  • Auditory comprehension
  • Oral expression
  • Reading comprehension
  • Written comprehension
  • Swallowing
  • Problem solving and memory

Social Worker Services

Social workers are not supplied directly from AHCP

Social Worker Services are a vital component of our home health care program. Our goal is to improve or maintain the social, emotional, functional, and physical health status of each client while enhancing the coping skills of the family and other caregivers. AHCP provides accredited Masters level social workers that assist clients and their families in care planning and coping mechanisms to enhance the home environment.

The role of a social worker includes:

  • Liaison with other members of the health care team
  • Advocacy with community agencies
  • Information and referral to programs such as Meals on Wheels
  • Counseling and crisis intervention
  • Assistance with long-term care planning
  • Fairfax County. Louding County and Prince William MSW or Social Workers afflication.

Certified Nurse Aides/Home Health Aides

Our aides assist the elderly, convalescent, or disabled to live in their own homes instead of a health care institution or facility. They provide healthcare and personal care services to clients, and are supervised by health professionals such as registered nurses.

They may:

  • Check clients’ pulse rates, temperatures, and respiratory rates
  • Assist with simple prescribed exercises
  • Keep clients’ rooms neat; and help clients move to and from bed, bathe, dress, groom, etc.

Specialty Nursing Services

An inter-disciplinary team of professional nurses who are experts in their fields provides AHCP Specialty Nursing Services. Their unique, specialized training enables AHCP to provide a comprehensive array of skilled nursing services, including direct clinical care and the education and counseling of our clients and their families. At AHCP we place great emphasis on developing and maintaining specialty services for our clients in order to provide an efficient and cost effective alternative to hospitalization or institutional care.

    • Diabetic Management Services

Clinical Case Management

American Health Care Professionals provides comprehensive, administrative, and clinical case management services with competitive rates, while maintaining and enhancing quality care.

Clinical Case Management includes:

  • Registered nurse supervision on call 24 hours/day
  • Updated and frequent client nursing care reports
  • Nursing care follow-ups
  • Provision of written plans of care
  • Client education
  • Clinical in-services
