Overview

When you are ill or recovering from an injury, there is no place you would rather be than home with people you love. Clients of American Health Care Professionals benefit from a comprehensive array of home health care services including diabetes management, cancer care, cardiopulmonary services, wound care, pre- and post-joint replacement care, and rehabilitation services following injury or stroke. AHCP also provides nursing care and rehabilitation following a hospital stay, and assistance with activities of daily living (ADLs) such as bathing, preparing meals, or light housekeeping.

Home health care is the foundation of what we do at American Health Care Professionals. Our agency can be your single source for complete home health care. Our home health care team includes registered nurses, diabetic educators, certified nurse aides and home health aides and rehabilitation therapists. We offer individualized services and work closely with the family and health professionals to develop an appropriate Care Plan.

Psychiatric Mental Health Nursing is a specialty within nursing. Psychiatric mental health registered nurses work with doctors, individuals, families, groups, and communities, assessing their mental health needs. AHCP uses nursing mental health diagnosis to create an individuals plan of care. This is implements into the clients medical home model to evaluates it effectiveness.
