Registered Nurse (RN)

Job Summary: Demonstrate proficient skills using OASIS to admit, transfer, re-certify, and discharge home health clients following physician orders; coordinate and supervise Licensed Practical Nurses (LPN) and Home Health Aides (HHA) in the delivery of client care; maintain compliance with agency policies and procedures; follow Medicare and Medicaid regulations.

Qualification Requirements: Must be a Board certified RN; must have a current license and have a minimum of one year nursing experience as RN; good work ethic and efficiency in service delivery and interactions with patients, families, and staff.

Supervision: Report to Case Manager/Director of Nursing (DON).


Licensed Practical Nurse (LPN)

Job Summary: Provide nursing care to patients in the home setting; observe and assess the client and/or caregiver to enhance the quality of life; demonstrate individualized creativity in educating the patient and/or caregiver; follow nursing policies and procedures per agency standards; follow the plan of care according to physician orders; demonstrate understanding of Medicare and Medicaid regulations.

Qualification Requirements: Must be a Board certified Licensed Practical Nurse, must have a current license and have a minimum of two years work experience as an LPN in a healthcare setting; and demonstrated oral and communication skills.

Supervision: Report to RN/Case Manager


Physical Therapist (PT)

Job Summary: Provide skilled therapy intervention and treatment to clients in a home setting to include appropriate utilization of treatment plans and assessment skills; monitor carefully the client’s status and progress toward goals as well as compliance with physician’s orders; and be responsible for educating patients and caregivers while preparing the patient/caregiver for discharge, as appropriate.

Qualification Requirements: Successful completion of a physical therapy program at a school of physical therapy approved by the American Physical Therapy Association or the Council on Medical Education and Hospitals or American Medical Association, or both; and if licensed or registered prior to 1966, meet the alternative qualifications cited in the Medicare Conditions of Participation, Section 405.12; must have a current license as a physical therapist; and must have a minimum of two years experience as a physical therapist.

Supervision: Report to the Nursing Supervisor/Case Manager


Occupational Therapist (OT)

Job Summary: Assist the attending physician in evaluating the client to determine his/her functional level; develop a plan of treatment (POT) to deliver services in accordance with the approved POT; guide the client in therapeutic, creative, and self-care activities.

Qualification Requirements: Graduate of an OT curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association or eligibility for the National Registration Examination of the American Occupational Therapy Association; must have a minimum of two years experience as an occupational therapist; must have a current license.

Supervision: Report to RN Supervisor/Case Manager


Speech Therapist (ST)

Certified Nurse Aide/Home Health Aide

Job Summary: Provide personal care and respite care services to clients in their homes (e.g., assistance with personal hygiene, shower, tub, or bed baths, grooming, transfer in and out of bed, meal preparation and feeding, when required); taking and recording blood pressure, pulse and respiration; and medication reminders.

Qualification Requirements: Must have completed training as certified nurse aide; must have a current license; or if a home health aide, a certificate from a DMAS-approved training school is required.

Supervision: Report to RN/LPN/PT/OT/ST


Social Worker (MSW)

Qualification Requirements: Must be licensed and have a master’s degree from a school of social work accredited by the Council on Social Work Education; must have one year experience of social work in a health care setting.

Supervision: Report to Case Manager/DON
