American Health Care Professionals (AHCP), established in 1995, is a home health care agency located in Great Falls, Virginia. AHCP is accredited by The Joint Commission. Our company provides a comprehensive, patient-centric model inclusive of: Home Health, Telehealth, Mobile Health, Global Health, and Wellness Programs.

AHCP empowers individuals, caregivers, and communities to understand and better manage their health. We strive to keep people in their homes by providing compassionate, high-quality services and emerging technology to make health care affordable for those in need.
Home Health
It is extremely exciting for us to be one of the first companies in the DC Metropolitan area to offer Telehealth and Mobile Health (mHealth) management and monitoring services.
Telehealth Services
AHCP Telehealth Remote Monitoring Services (RMS) utilizes individual AHCP HEALTHwins™ devices and a multiuser AHCP HEALTHwins™ kiosk to monitor client health information.
Prevention Wellness
AHCP Wellness Improvement Network (W.I.N™) teams are champions in the implementation of prevention and wellness programs for both non-profit and profit centered organizations.
Global Partners
AHCP Global Health Care partner’s with countries, communities and organizations to offer telehealth, mobile health, prevention and wellness, wellness medical records (WMR) and health care consultation.
